To schedule a new client discovery phone call or cancel/reschedule an appointment for an existing client, please use the button below. Existing clients will be redirected to sign into their client portal account.

Discovery Call requests are typically confirmed early in the week in which they are scheduled. For example, if discovery calls are booking out five weeks, you may not receive a confirmation email until early in that week. This ensures information collected via patient portal forms will be current. If a conflict arises with the selected date/time, you will be notified as soon as possible and provided alternate dates/times within the week of the initially scheduled call. Discovery calls take place by phone, using the phone number provided in the portal. Please disregard any video telehealth links that you may receive.
